Qualifications

  • Bachelor’s Degree in Social Work.
  • Minimum two (2) years of work experience in case management OR
  • Minimum two (2) years of direct service to the targeted population or similar experience
  • Strong understanding and sensitivity to diverse cultures and lifestyles, with demonstrated experience working effectively with Alaska Native population.

Responsibilities

  • Assess consumer needs based on their needs, limitations, and desires.
  • Develop a case plan with the consumer.
  • Maintain constant communication with the consumer.
  • Maintain consumer’s confidence and protect operations by keeping information confidential.
  • Coordinate and provide services that are client centered.
  • Provide trauma-informed, culturally competent holistic services to victims.
  • Develop and maintain client case records.
  • Handle case assignments, drafts service plan, reviews progress and determines closure.
  • Work with consumers and community agencies to coordinate and facilitate services.
  • Advise consumers to help them make informed decisions.
