Responsibilities

• Function as a member of an interdisciplinary health care team.
• Conduct brief psychosocial and health needs assessments (AUDIT, PHQ, GAD, etc.).
• Provide brief counseling on tobacco cessation, substance use, and health interventions.
• Collaborate with patient and family to create an appropriate care plan.
• Develop referral, tracking and follow-up plans for care coordination.
• Document information per protocol in the EHR system and discuss care options.
• Engage with community shelters to motivate clients to access services.
• Maintain community referral systems and participate in QA activities.
