Job summary

Denali Family Services in Anchorage is seeking an Early Childhood Case Manager to support children and families with community-based behavioral health services. You will coordinate care across home, school, and community settings to promote stability and growth for youth experiencing severe emotional disturbance (SED).

Work closely with clinicians, educators, and partners to provide individualized supports that help families navigate services and access resources.

Qualifications

• Bachelor's degree required.
• Minimum of 1 year of care coordination or direct services experience.
• Experience with developmental disabilities and/or severe emotional disturbance preferred.
• Strong communication, documentation, and organizational skills.
• Ability to collaborate effectively within a multidisciplinary team.

Responsibilities

• Coordinate services for youth and families across home, school, and community settings.
• Develop and monitor individualized service plans and transition plans.
• Collaborate with families, schools, clinicians, and partners to ensure coordinated care.
• Assist youth and families in accessing community resources such as education and housing.
• Facilitate communication among service providers to ensure continuity of care.
• Complete timely and accurate documentation per agency standards.
• Participate in treatment planning and skill development as part of the care team.
• Transport clients to appointments or services when necessary.
