Job summary

Sheboygan County Health & Human Services seeks a dedicated Case Manager to provide comprehensive case management and service coordination for individuals, children, youth, and families. You will assess needs, determine eligibility, develop service plans, and coordinate across systems to connect clients with the right resources.

Under supervision, you will monitor outcomes, maintain records, and collaborate with crisis services, schools, and community partners.

Qualifications

• Bachelor’s degree from an accredited college in Social Work, Human Services, Behavioral Science, or closely related field.
• Relevant experience in social services, case management, or service coordination preferred.
• Valid Wisconsin Driver’s license.

Responsibilities

• Provide comprehensive case management and service coordination for individuals, children, youth, and families with diverse needs.
• Assess needs, determine eligibility, develop and implement service plans, and coordinate across systems.
• Monitor outcomes to support safety, stability, recovery, and well-being.
• Maintain complete case records and documentation in electronic and paper systems.
• Collaborate with crisis services, schools, courts, and community partners for coordinated delivery.
• Engage in trauma-informed, culturally responsive, strengths-based practices.
