Job summary

Sheboygan County Health and Human Services is seeking a Case Manager to provide comprehensive case management and service coordination for individuals, children, youth and families with diverse needs. Qualifications

  • Bachelor’s degree in Social Work, Human Services, Behavioral Science, or closely related field.
  • Relevant experience in social services, case management, service coordination preferred.
  • A valid Wisconsin driver’s license required.

Responsibilities

  • Plan, provide, arrange, and monitor direct client services under supervision.
  • Provide information, outreach, and assistance; assess eligibility and need; initiate referrals.
  • Conduct comprehensive assessments of individuals and families to evaluate strengths and needs.
  • Provide case management and service coordination for clients with mental health, SUD, developmental, or other challenges.
  • Develop outcome-focused service plans with clients and supports; secure needed services.
  • Coordinate interdisciplinary meetings with clients, families, schools, and partners.
  • Monitor and adjust services to meet needs and funding requirements.
  • Maintain complete, timely case records and documentation across systems.
  • Ensure knowledge of and compliance with federal/state laws, rules, and standards.
  • Advocate for clients to promote access to services and reduce barriers.
  • Collaborate with crisis services, law enforcement, healthcare providers, and emergency systems.
  • Travel within the County as required and maintain a valid driver's license.
