Job summary

Sheboygan County Health & Human Services is seeking a Case Manager to deliver comprehensive case management and service coordination for individuals, children, youth, and families with diverse needs. The role emphasizes trauma-informed, culturally responsive practices and outcomes-focused planning.

Under supervision, you will assess needs, develop service plans, coordinate resources, and monitor progress while ensuring compliance with laws and funding. Hybrid work options may be available.

Qualifications

• Bachelor’s degree in Social Work or related field.
• Experience in social services or case management preferred.
• Ability to travel within/outside the county with a valid driver’s license.
• Knowledge of trauma-informed care and cultural humility.

Responsibilities

• Provide comprehensive case management and service coordination for individuals, children, youth, and families with a wide range of needs.
• Develop comprehensive, outcome-focused service plans and coordinate services across systems.
• Monitor and adjust services to ensure alignment with client needs and funding requirements.
• Maintain accurate case records and documentation in designated systems.
• Collaborate with crisis services, law enforcement, healthcare providers, and community partners.
