Job summary

Sheboygan County Health and Human Services is seeking a Case Manager to provide child welfare and family services within the Social Services Division, focusing on CPS and related programs. Under supervision, you will assess safety, risk, and family needs, coordinate services, and work with families, courts, and partners to promote safety, permanency, and stability.

Qualifications

• Bachelor’s degree from an accredited college or university in Social Work, Human Services, Behavioral Science, or closely related field.
• Experience in social services, case management, service coordination, or a comparable human services role serving children, families, or individuals with disabilities is preferred.
• Certified Social Worker (CSW) licensure or an active Social Worker Training Certificate is preferred.
• A Valid Wisconsin Driver’s license.

Responsibilities

• Plan, provide, arrange, and monitor direct client services under supervision within assigned Social Services areas.
• Provide information, intake, outreach, and assistance to individuals, families, mandated reporters, and the public; gather and evaluate information to determine service needs.
• Receive and respond to reports of alleged child abuse or neglect and conduct CPS investigations and family assessments.
• Determine safety threats, implement and monitor safety plans in accordance with Wisconsin statutes and policies.
• Provide case management and service coordination for children and families involved with CPS and other Social Services programs.
