Job summary

Colorado Coalition for the Homeless is seeking a Case Manager I to support families and individuals experiencing homelessness by developing care plans and coordinating services.

You will work with integrated health teams, community partners, and case managers to deliver outreach and crisis intervention, documenting progress in the EHR.

Qualified candidates have a high school diploma or GED, a Colorado driver's license, and 1–2 years of relevant experience; a bachelor's degree is preferred.

Qualifications

• Requires high school diploma or GED; bachelor’s degree preferred.
• BLS certification within 30 days of employment.
• Colorado driver’s license within 30 days of hire.

Responsibilities

• Manage a caseload of clients with high service needs.
• Coordinate services, monitor progress, and document outcomes.
• Provide crisis intervention and safety planning.
• Develop referrals and partner with community resources.
• Maintain accurate documentation in the EHR system.
• Transport clients as needed.
