Job summary

Qualifications

• Required: High School Diploma/GED and at least six years of case management or related services.
• Preferred: Bachelor's degree in Social Work, Human Services, or related field.
• Experience with trauma-informed care and youth-focused services is highly desirable.

Responsibilities

• Maintain a caseload of 7–8 participants and provide trauma-informed case management.
• Conduct intake assessments and develop Sustainable Growth Plans.
• Coordinate referrals to behavioral health, housing, legal services, and other supports.
• Document accurately in Salesforce and other databases.
