Job summary

Good Shepherd Services in New York City offers a Case Manager (Credible Messenger) role providing intensive, trauma-informed case management and supportive services to at-risk young adults. You will build trusting relationships, coordinate resources, and promote long-term stability in collaboration with program leadership.

The position reports to the Program Director, BRAG WEST/RESTORE, requires 35 hours weekly, non-exempt, and a salary range of $48,000–$53,000.

Qualifications

• High School Diploma/GED & minimum of six years' experience in case management or related services.
• Preferred: Bachelor's degree in Social Work, Human Services, psychology or related field.
• Experience with justice-involved or high-risk youth is preferred.
• Knowledge of trauma-informed care, crisis intervention and community resources.
• Strong assessment, organizational, documentation and communication skills.
• Ability to travel throughout NYC and work flexible hours.
• Credible Messenger experience is strongly preferred.
• Experience with Motivational Interviewing and restorative practices.
• Familiarity with the Bronx and surrounding communities.
• Driver license preferred.

Responsibilities

• Maintain a caseload of approximately 7–8 participants.
• Conduct trauma-informed intake assessments using ARC criteria.
• Develop Sustainable Growth Plans and monitor participant progress.
• Conduct weekly case management visits across office, home, hospital and community.
• Build trusting relationships through mentorship, advocacy and support.
• Coordinate referrals to behavioral health, housing, healthcare and services.
• Advocate with community agencies and service systems for participants.
• Document progress, safety plans, and timesheets accurately.
• Support supervision, meetings and program events.
• Deliver workshops and psychoeducational groups focused on growth.
