Job summary

Region Ten CSB is seeking a Case Manager to provide assessment, case management, psycho-education, and rehabilitative services to adult women in the Project Link Program in Charlottesville, VA. Qualifications

• Bachelor’s degree in Social Work, Psychology, Sociology or Human Services; Virginia RN licensure or CSAC/CSAC Assistant certification may be acceptable.
• One year of experience with substance use disorders or co-occurring mental health disorders is preferred.
• Valid Virginia driver’s license with acceptable driving record and valid auto insurance for business use.

Responsibilities

• Provide assessment, case management, and referrals to community resources to meet recovery-oriented and basic needs (food, housing, employment, parenting, health).
• Remain an active member of the Project Link team and participate in monthly client meetings.
• Coordinate linkage to individual and group therapeutic services and collaborate with treatment programs involved in care.
• Provide crisis intervention as needed and conduct home visits to fulfill face-to-face requirements.
• Transport clients as necessary and support program expansion with supervision from leadership.
• Attend or coordinate treatment team meetings and serve as a resource consultant for current participants.
• Maintain paperwork and cross-agency communications, and complete required trainings.
