Job summary

Region Ten CSB in Charlottesville, VA is seeking a professional Case Manager (FLSA non-exempt) to provide Care Coordination for adults with mental health and/or substance use concerns, including those connected with the Criminal Justice System and re-entry planning.

You will develop access plans, liaise with consumers, families, programs and providers, and monitor progress in collaboration with the supervisor.

Qualifications

• Bachelor’s degree in a Social Work, Psychology, Sociology, or Human Services field.
• QMHP certification is required.
• 1 year of relevant work experience preferred (mental health/substance use).

Responsibilities

• Provide care coordination for adults 18+ with mental health and/or substance use concerns, including those in the Criminal Justice System.
• Develop access to services and re-entry planning; liaise with consumers, families, programs and providers.
• Monitor progress, advocate for clients, and educate consumers and families.
• Maintain complete and accurate records in compliance with agency and state standards.
• Coordinate services across providers and human service systems; ensure releases are in place.
• Engage with community resources to promote habilitation and community integration.
• Assist with transitions and linkage to support services in the community and jail settings.
• Collaborate with supervisors and program directors to meet client needs.
