Job summary
Hampton-Newport News Community Services Board seeks a Case Management Supervisor for Project Link to oversee day-to-day case management for adults and mothers with children affected by substance use and co-occurring disorders.
You will supervise Case Managers and Peer Recovery Specialists, coordinate assessments and service planning, and partner with healthcare providers and community services to support recovery and community integration.
Qualifications
- Bachelor's degree in Human Services (Social Work, Psychology, Sociology, or Counseling).
- At least 3 years of case management experience with individuals living with a substance use disorder.
Responsibilities
- Supervise Case Managers and Peer Recovery Specialists.
- Oversee assessments and individualized service planning.
- Coordinate referrals and community resources.
- Support crisis intervention and continuity of care.
- Balance caseloads and provide staff training and performance support.
- Collaborate with healthcare providers, courts, and community partners.
- Monitor program expenditures and outcomes; recommend improvements.
