Job summary

Qualifications

• Bachelor's degree in a Social Work, Psychology, Sociology, and/or Human Services field.
• A minimum of 1 year of work experience; preferably with individuals with substance-use disorder or mental health disorders.
• QMHP certification is required; Virginia Driver’s License with valid driving record.

Responsibilities

• Enhancing community integration through increased opportunities for community access and involvement.
• Making collateral contacts to promote implementation of individualized services plan.
• Coordinate release planning and linkages with community supports to promote habilitation and goals.
• Maintain complete and current consumer records in compliance with standards.
• Advocate for individuals in response to changing needs.
