Job summary

ServiceSource is seeking a dedicated Case Manager to provide diverse employment services for veterans facing disability, poverty, homelessness or justice involvement in Colorado. You will coordinate vocational and pre-employment supports, conduct needs assessments, and guide participants toward sustainable job goals with resources and referrals.

The role includes intake, documentation, and collaboration with partners to address barriers, while promoting access to benefits and community services

Qualifications

• Excellent Microsoft Office skills, especially Excel, Outlook, and Word.
• Ability to work in multiple databases and proficient in data entry.
• Excellent communication skills (written and verbal).

Responsibilities

• Serve as Case Manager within the employment team, providing a range of services and referrals to meet participant needs.
• Provide support and advocacy to maintain access to public benefits and community resources.
• Educate individuals on how income may affect other benefits they may receive.
• Assist participants in navigating barriers and address hurdles such as missed court dates or active warrants.
• Complete all documentation and data entry in a timely manner.
• Act as intake professional, create files, conduct initial assessments, and set case management plans.
• Update files as milestones are met and adapt to new needs.
• Collaborate with a wide range of stakeholders and manage shifting priorities.
