Job summary

ServiceSource is seeking a dedicated Case Manager to support veterans experiencing disability, justice involvement, poverty, and homelessness through employment programs in Colorado. You will coordinate vocational and pre-employment services, conduct assessments, and guide participants toward sustainable employment within a trauma-informed framework.

Responsibilities include intake, data entry, and collaboration with partners to remove barriers, while advocating for benefits and resources.

Qualifications

• High school diploma or GED required with 2 years of case management experience.
• Experience in crisis intervention and trauma-informed care related to poverty, homelessness, and disability is preferred.
• Veterans are strongly encouraged to apply.

Responsibilities

• Serve as a case manager within the employment team, coordinating services and referrals to meet participant needs.
• Provide advocacy to maintain access to public benefits and community resources for participants.
• Educate participants on how income affects other benefits they may receive.
• Assist participants in addressing barriers and completing documentation in a timely manner.
• Act as intake professional, create files, conduct initial assessments, and set up case management plans.
