Job summary

Shepherd’s House Ministries seeks a compassionate Case Manager to support individuals facing homelessness and related challenges across our Bend sites. You will deliver trauma-informed, participant-centered services, coordinate housing referrals, and help individuals progress toward stable housing and well-being.

The role emphasizes collaboration with staff, volunteers, and community partners, maintaining documentation, and upholding ethical standards in all service settings.

Qualifications

• Experience providing trauma-informed case management to individuals experiencing homelessness or behavioral health challenges.
• Ability to assess needs, develop service plans, coordinate referrals, and maintain confidential documentation.

Responsibilities

• Manage an assigned participant caseload with trauma-informed, strengths-based case management and individualized service planning.
• Maintain participant engagement through follow-up, coaching, advocacy, and crisis response.
• Connect participants to housing resources and stability pathways and coordinate with housing providers.
• Coordinate referrals to internal and external services for continuity of care.
• Monitor progress, adjust service plans, and identify barriers or gaps.
• Maintain accurate, confidential documentation in HMIS and other systems.
