Job summary

Shepherd’s House Ministries seeks a compassionate Case Manager to support individuals facing homelessness and related challenges across our Bend sites. You will deliver trauma-informed, participant-centered services, coordinate housing referrals, and help individuals progress toward stable housing and well-being.

The role emphasizes collaboration with staff, volunteers, and community partners, maintaining documentation, and upholding ethical standards in all service settings.

We are seeking a passionate and mission-driven Case Manager to help expand the impact of Shepherd’s House Ministries across all sites and advance this life-changing work.

The Case Manager is accountable for providing consistent, trauma-informed, participant-centered support that promotes safety, stability, and progress toward housing and personal well-being. This role is responsible for engaging participants, assessing needs, developing and monitoring individualized plans, coordinating access to appropriate resources and services, maintaining accurate and confidential documentation, and collaborating effectively with staff, volunteers, and community partners. The Case Manager upholds organizational values, ethical standards, and compliance requirements while contributing to a safe, respectful, and healing environment across all service settings.

Essential Duties and Responsibilities:
  • Manage an assigned participant caseload, providing trauma-informed, strengths-based case management, outreach, assessments, and individualized service planning.
  • Maintain consistent participant engagement through follow-up, coaching, advocacy, accountability, and crisis response when needed.
  • Connect participants to housing resources and stability pathways, including applications, referrals, and coordination with housing providers.
  • Coordinate and facilitate referrals to internal and external services to ensure continuity of care and comprehensive support.
  • Monitor participant progress, adjust service plans as needs change, and identify barriers or service gaps.
  • Maintain accurate, timely, and confidential documentation in HMIS and other required systems, including facility-level reporting.
  • Provide a welcoming, safe, and supportive environment across Navigation Services sites and outreach settings.
  • Support daily site operations, including participant activities, volunteer engagement, facility safety, and supply coordination.
  • Collaborate with internal teams, volunteers, and community partners to improve service delivery and address chronic houselessness.
  • Uphold organizational policies, ethical standards, and mission while contributing to team meetings, trainings, and program effectiveness.
Qualifications:
Education & Experience:
  • Bachelor’s degree in Social Work, Human Services, Psychology, Behavioral Sciences, or a related field, or equivalent combination of education and relevant lived/professional experience preferred
  • Experience providing trauma-informed case management and support to individuals experiencing homelessness or complex behavioral health and substance use challenges.
  • Ability to assess needs, develop service plans, coordinate referrals, and maintain accurate, confidential documentation in systems like HMIS.
  • Strong communication, crisis de-escalation, and organizational skills; able to work independently or on multidisciplinary teams with flexible scheduling.
Working Conditions/Physical Requirements:
  • Ability to sit, stand, walk, and move throughout facilities and community locations for extended periods
  • Ability to transition frequently between administrative tasks (computer/data entry) and direct participant interaction
  • Ability to bend, reach, stoop, and lift up to approximately 25 pounds on an occasional basis
  • Ability to use a computer, phone, and other standard office equipment for documentation, communication, and reporting
  • Ability to travel locally between program sites or outreach locations, as required by the role
  • Ability to visually and audibly assess situations, read documentation, and communicate clearly with participants, staff, and partners
