Job summary

Shepherd's House Ministries in Bend, Oregon is seeking a passionate Case Manager to support trauma-informed, participant-centered services across our sites, advancing housing stability and personal well‑being.

You will assess needs, develop individualized plans, coordinate referrals to housing resources, maintain confidential HMIS documentation, and collaborate with staff, volunteers, and partners to expand the organization's impact.

Qualifications

• Trauma-informed case management experience.
• Strong communication, crisis de-escalation, and organizational skills.
• Ability to assess needs, develop service plans, and coordinate referrals.

Responsibilities

• Manage an assigned participant caseload with trauma-informed, strengths-based case management.
• Maintain participant engagement through follow-up, coaching, advocacy, and crisis response.
• Connect participants to housing resources and stability pathways, including applications and referrals.
• Coordinate and facilitate referrals to internal and external services to ensure continuity of care.
• Monitor progress, adjust service plans as needs change, and identify barriers to care.
• Maintain confidential documentation in HMIS and other required systems.
• Provide a welcoming, safe environment across Navigation Services sites and outreach settings.
• Support daily site operations, volunteer engagement, and collaboration with community partners.
