Job summary

Brooklyn Community Services is seeking a Case Manager for the Family Shelter to provide trauma-informed, family-centered case management, stabilization, and housing placement services at our Brooklyn shelter. You will develop individualized service plans, connect families to benefits and housing programs, document work in CARES, and participate in supervision and team meetings.

A Bachelor's degree is required; MSW preferred; experience in shelter or family services is strongly preferred;

Qualifications

  • Bachelor's degree in Social Work, Human Services, Psychology, or related field required.
  • MSW preferred.
  • Minimum of one year of case management experience.
  • Familiarity with NYC housing resources and DHS CARES is a plus.
  • Bilingual ability preferred.

Responsibilities

  • Manage a caseload of families and conduct regular meetings to assess needs.
  • Develop individualized Service Plans addressing housing, benefits, childcare, health.
  • Connect families to housing subsidies, benefits, legal services, and mental health supports.
  • Document cases in CARES and prepare required progress reports.
