Job summary

Brooklyn Bureau of Community Service (inc) seeks a Case Manager to provide trauma-informed, comprehensive case management for families in shelter. You will support stabilization, develop individualized service plans, and coordinate housing and support services to move families toward permanent housing and self-sufficiency.

Responsibilities include managing caseloads, linking to benefits, housing applications, and ensuring documentation in CARES; requires a bachelor’s degree in a related field

Qualifications

• Education: Bachelor's degree in Social Work, MSW preferred.
• Experience: Minimum of one year of case management experience in shelter, housing, or family services.
• Skills: Interpersonal, written, and organizational skills; trauma-informed practice.

Responsibilities

• Carry an assigned caseload of families; conduct regular individual and family meetings to assess needs, monitor progress, and adjust service plans.
• Develop individualized Service Plans with families within required timeframes, addressing housing, income, benefits, childcare, health, and other identified needs.
• Connect families to internal and community-based resources including benefits, employment, legal services, childcare, and mental health supports.
• Assist families in completing housing applications, documentation, and eligibility processes for housing subsidy programs.
• Conduct room visits, family meetings, and required DHS contacts on schedule; maintain documentation in CARES.
