Job summary

Brooklyn Community Services (BCS) is seeking a Case Manager for the Family Shelter program. The role focuses on trauma-informed case management to support families in stabilizing immediate needs and pursuing permanent housing.

You will work with shelter staff to develop individualized service plans, connect clients to housing resources, benefits, and mental health supports, and ensure accurate documentation in CARES and DHS systems. Strong communication and collaboration are essential.

Qualifications

• Education: Bachelor’s degree in Social Work, Human Services, Psychology, or related field required; MSW preferred.
• Experience: Minimum of one year of case management experience; shelter, housing, or family services preferred.
• Knowledge: Familiarity with NYC housing resources, public benefits, and DHS CARES system a plus.

Responsibilities

• Carry an assigned caseload of families; conduct regular meetings to assess needs and adjust service plans.
• Develop individualized Service Plans within required timeframes covering housing, income, benefits, childcare, health, and other needs.
• Connect families to benefits, employment, legal services, childcare, and mental health supports.
• Assist families in completing housing applications, documentation, and eligibility for housing subsidy programs.
• Maintain documentation in CARES and DHS systems; participate in supervision and team meetings.
