Job summary

Community Action Agency of Oklahoma City & OK/CN Counties, Inc. provides a Case Manager role to work with Will Rogers Courts residents, guiding them through renovation and relocation with trauma‑informed, individualized support.

You will conduct intake, assess needs, and develop Family Service Plans that target stable housing, employment, education, and health access. You will collaborate with partners, monitor progress, and connect families to resources while maintaining detailed documentation

Qualifications

• Associate or higher degree in a related field is required; Bachelor's preferred.
• Minimum of 2 years in case management or related direct-service work.
• Experience with trauma-informed care is preferred.

Responsibilities

• Conduct intake interviews using standardized forms to determine household needs.
• Administer needs assessment tools to identify priorities and urgency.
• Enter and maintain family data in the case management system.
• Provide emergency referrals for food, homelessness prevention, and mental health.
• Develop Family Service Plans with SMART goals.
• Coordinate referrals across housing, job training, healthcare, and education.
• Hold weekly check-ins and conduct biweekly or monthly case reviews.
• Track progress and document services accessed; participate in quarterly impact analyses.
• Support families during renovation and relocation with continuity of care.
