Job summary

Crossroads Health in Northeast Ohio seeks a Case Manager for North Coast House to provide trauma-informed case management in residential and community settings. You will help clients and families with needs assessment, referrals, and group services, including follow-up after discharge.

The role emphasizes personal transportation, driving within Lake County, and delivering services that support transition and recovery for adults in our community.

Qualifications

• High School Diploma or equivalent required.
• Associate’s/Bachelor’s in Human Services/Social Work or related field preferred.
• QMHS/QBHS within 15 days of hire (employer provided).
• Must have a driving record acceptable for transporting clients and carry auto insurance with standard limits.
• Willingness to travel within Lake County and surrounding counties and transport clients in own car.

Responsibilities

• Provides therapeutic interventions in the home or community and links clients to resources.
• Assesses clients’ development, psychological and psychiatric needs to make referrals.
• Advocates on behalf of clients.
• Monitors clinical symptoms.
• Facilitates group sessions.
• Acts as gatekeeper for other resources.
• Performs crisis intervention as needed.
• Meets monthly productivity metrics.
