Job summary

Crossroads Health in Northeast Ohio is seeking a dedicated Case Manager for North Coast House. You will provide trauma-informed case management services in residential and community settings, linking clients to resources and supporting transition after discharge.

Responsibilities include home visits, group services, crisis intervention as needed, and meeting monthly productivity metrics. A High School Diploma or higher is required, with preferred Associate’s or Bachelor’s in Human Services or

Qualifications

• High School Diploma or equivalent.
• Associate’s Degree/Bachelor's Degree in Human Services/Social Work or related field (preferred).
• Three years’ related experience with High School Diploma or equivalent, or Associate’s Degree.
• QMHS/QBHS within 15 days of hire (employer provided).
• Must have an acceptable driving record 4 (four) points or less, reliable transportation and carry automobile insurance with minimum liability of $100,000/$300,000, and property damage of $50,000.
• Work in home and community. Travel within Lake County, providers to adult clients also travel to surrounding counties as needed. Must be comfortable driving on the highways. Must be comfortable making home visits throughout the community and transporting clients in own car.

Responsibilities

• Provides therapeutic interventions in the home or community and linking clients to resources.
• Assesses clients’ development, psychological and psychiatric needs in order to make referrals.
• Advocates on behalf of clients.
• Monitors clinical symptoms.
• Facilitates group sessions.
• Functions as a gatekeeper for other resources.
• Performs crisis intervention, as needed.
• Meet monthly productivity metrics.
