Job summary

Crossroads Health in Northeast Ohio seeks a Case Manager to support clients in home or community settings, linking them to critical resources and advocating for their needs.

You will assess developmental, psychological, and psychiatric needs, coordinate referrals, monitor symptoms, and meet monthly productivity targets while upholding the organization's commitment to equity and high-quality care.

Qualifications

• High School Diploma with mandatory 3 years experience in a behavioral/mental health setting.
• Associates degree is highly desirable.
• Demonstrated passion to advocate for others.
• Demonstrated self-starter and independent worker.
• Must have an acceptable driving record and carry automobile insurance with adequate liability limits.

Position Requirements:

• High School Diploma with a mandatory 3 years experience working within a behavoral/mental healtth setting. Associates degree is highly desirable.
• QMHS/QBHS within 15 days of hire (employer provided)
• Demonstrated passion to advocate on behalf of others
• Demonstrated self-starter and independent thinker who can work in the field as well as in the office
• Organized with good time management skills necessary
• Must have an acceptable driving record 4 (four) points or less, reliable transportation and carry automobile insurance with minimum liability of $100,000/$300,000, and property damage of $50,000.

***The anticipated hiring range for this position is $20.00 - $23.00 per hour. Starting pay will be commensurate with experience, qualifications, and internal equity.
