Job summary

INcompass Healthcare in Indiana seeks a Case Manager to provide skills training and case management services to identified clients, delivering therapeutic services per the service plan and collaborating as a key member of the treatment team.

The role requires compassion, trauma-informed care, crisis management, strong communication, and the ability to coordinate services with internal and external programs. Travel to client homes and community settings is expected.

Qualifications

• Associate degree or equivalent experience in human services or related field.
• Strong written and verbal communication skills with clients and families.
• Ability to work independently within a treatment team.
• Demonstrates organizational and time-management abilities.
• Ability to travel to client homes and community settings.
