Job summary

LifeBridge Health's Center for Hope seeks a Case Manager to partner with clients and families, guiding them toward personal goals. You will assess needs, link to services, and provide warm hand-offs, while also conducting home visits and facilitating literacy, job readiness, and resilience workshops.

Responsibilities include coordinating service applications, conducting seminars, and building community partnerships to promote programming.

Qualifications

• Bachelor's degree required in Social Work, Human Services, Family Studies, Psychology or similar field.
• 1–3 years of experience in case management or related social services.

Responsibilities

• Provide case management services to clients and families in the Center for Hope program.
• Receive referrals and conduct initial needs assessments.
• Partner with clients on case planning and goal setting, tracking progress.
• Vet community programs to ensure trauma-informed, culturally appropriate service delivery.
• Assist with completing service and program applications and conduct home/community visits.
• Engage clients with empathy and compassion; conduct support groups and seminars on literacy, job readiness, and resilience.
• Identify client needs and develop curricula for support groups and monthly seminars.
• Conduct client seminars on resilience, school engagement, family engagement, coping, life skills, and more.
• Provide community outreach, engagement, and education; present trainings to community partners.
