Job summary

Health Care for the Homeless in Baltimore is seeking a Therapist Case Manager to provide mental health therapy and intensive case management for adults experiencing homelessness. You will partner with clients to support health, housing stability, and personal goals in a trauma-informed, client-centered approach.

Qualifications

• Master's degree in Social Work (LMSW or LCSW-C) required.
• At least 1 year of therapy and case management experience preferred.
• Knowledge of Harm Reduction, Motivational Interviewing, and Housing First.

Responsibilities

• Deliver behavioral health assessments, individual and group therapy, and intensive case management.
• Conduct home and community visits to monitor progress and well-being.
• Collaborate with interdisciplinary teams and external partners to coordinate care.
• Support budgeting and financial literacy as part of representative payee services.
• Maintain thorough and timely documentation in the electronic health record.
