Job summary

LifeBridge Health's Center for Hope seeks a Case Manager to partner with clients and families, guiding them toward personal goals. You will assess needs, link to services, and provide warm hand-offs, while also conducting home visits and facilitating literacy, job readiness, and resilience workshops.

Responsibilities include coordinating service applications, conducting seminars, and building community partnerships to promote programming.

Qualifications

  • Bachelor's degree required in Social Work, Human Services, Family Studies, Psychology or similar field.
  • 1–3 years of experience in case management or related social services.

Job description

The Case Manager directly partners with clients within various Center for Hope programs, providing support and guidance in reaching client goals. The Case Manager receives referrals from Center for Hope partners, including government and community agencies. This position is responsible for conducting client and family needs assessments, linking to indicated services or programs and providing a warm hand-off, helping clients to complete service and program applications, conducting home and community visits, conducting workshops and support groups, liaising between client and community resources, accompanying clients to court dates and other hearings, and conducting various forms of community outreach and education. The Case Manager is also responsible for other pertinent duties as assigned by the Program Manager.

KEY RESPONSIBILITIES:

  • The Case Manager will provide case management services to clients and families participating in the Center for Hope program.
  • Receive client and family referrals and conduct initial needs assessments.
  • Partners with clients in case planning and goal setting, continuously evaluating and recording progress made towards goals.
  • Assist in vetting various community programs and agencies to ensure a trauma-informed and culturally appropriate environment and service delivery.
  • Conduct other forms of case management, including: helping clients to complete various service and program applications, conducting home and community visits, engaging clients with empathy and compassion, etc.
  • The Case Manager will conduct support groups and seminars on literacy, job readiness, anger management, resilience, coping mechanisms, conflict resolution, family engagement, parent-child bonding, and other pertinent topics.
  • Identify areas of need and themes of client/family challenges and create curriculum for implementation in support groups and monthly seminars.
  • Conduct seminars for clients on topics such as resilience, school engagement, family engagement, coping mechanisms, literacy, job readiness, anger management, youth engagement, caregiver/child bonding, life skills, and more.
  • Provide community outreach, engagement, and education.
  • Conduct presentations and trainings to community partners - agencies and community members - on interpersonal dynamics, trauma, program subject matter expertise, trauma-informed practices, culturally appropriate responses.
  • Partner with Center for Hope programs to promote programming at community events and other gatherings.
  • Attend various community events on behalf of Center for Hope and build relationships with community members, agencies, and other stakeholders.

REQUIREMENTS:

  • Education: Bachelor's degree required in Social Work, Human Services, Family Studies, Psychology or other similar field
  • Experience: 1-3 years of experience

Pay Range: $23.75 to $39.12 Based On Experience
