LifeBridge Health's Center for Hope seeks a Case Manager to partner with clients and families, guiding them toward personal goals. You will assess needs, link to services, and provide warm hand-offs, while also conducting home visits and facilitating literacy, job readiness, and resilience workshops.
Responsibilities include coordinating service applications, conducting seminars, and building community partnerships to promote programming.
The Case Manager directly partners with clients within various Center for Hope programs, providing support and guidance in reaching client goals. The Case Manager receives referrals from Center for Hope partners, including government and community agencies. This position is responsible for conducting client and family needs assessments, linking to indicated services or programs and providing a warm hand-off, helping clients to complete service and program applications, conducting home and community visits, conducting workshops and support groups, liaising between client and community resources, accompanying clients to court dates and other hearings, and conducting various forms of community outreach and education. The Case Manager is also responsible for other pertinent duties as assigned by the Program Manager.

Pay Range: $23.75 to $39.12 Based On Experience
