Job summary

Children’s Crisis Treatment Center in Philadelphia seeks a Care Coordinator to support the Tamaa program, collaborating with a treatment team to address crisis and behavioral health needs for children ages 2-14 and their families. You will work trauma-informed with diverse communities and connect families to essential services.

The role requires a Bachelor's in Social Work, at least two years in case management with children and families, and fluency in relevant languages; strong community ties

Qualifications

• Bachelor's degree in Social Work or a related field.
• Minimum of two years' experience as a case manager or in a related role with children and families.
• Knowledge of African immigrant and refugee communities, cultural norms, traditions, and local resources.
• Strong ties to African immigrant, refugee, or diaspora communities; ability to build trust with children, families and community stakeholders.
• Fluency in Swahili, French, Amharic, Somali, Arabic, Lingala or other relevant languages preferred.
• Ability to work well in a multi-disciplinary team and with a culturally/ethnically diverse population.

Responsibilities

• Identify resources within the Philadelphia area (MH/MR centers, schools, day cares, health centers, housing programs) and maintain updated records.
• Establish and maintain relationships with West-African refugee communities and leaders.
• Assess refugee children and families and develop individualized service plans with families.
• Link families to needed resources and remove barriers to services (e.g., insurance).
• Conduct monthly face-to-face meetings to assess progress and update service plans.
• Collaborate with the team and external resources to ensure needs are met and interventions implemented.
• Maintain written/electronic records of client and resource contacts.
• Organize and document Multicultural Community Events and participate in trainings and staff meetings.
