Job summary

Tides Family Services, Inc. is seeking a Trauma-Focused Community-Based Services Care Coordinator to support children and youth who have experienced trauma, with emphasis on those at risk related to sexual abuse.

You will work collaboratively with families, schools and partners to promote holistic recovery in a community-based setting. Under supervision, you will deliver individual, group, and family services, participate in on-call rotations, and ensure accurate documentation in the EHR.

Qualifications

Bachelor's degree in Social Work, Psychology, or related field is required.
Minimum of three years of trauma-informed care experience.
Valid driver's license and reliable transportation.

Responsibilities

Deliver trauma-informed services to children and youth under supervision.
Provide psychoeducation to caregivers about impacts of sexual abuse.
Collaborate with clinicians, schools, and community partners for holistic support.
Participate in 24/7 on-call rotation for crisis intervention.
Maintain timely documentation in the agency's EHR system.
Support risk assessment, safety planning, and aftercare planning.
