Job summary

Children’s Crisis Treatment Center in Philadelphia seeks a Care Coordinator to support the Tamaa program and follow through with treatment team recommendations for refugee children aged 2–14 and their families in a trauma‑informed setting.

Identify community resources across Philadelphia, establish ties with West-African refugee groups, assess needs, develop service plans, link families to services, and maintain written and electronic records of contacts.

Qualifications

• Bachelor's degree in Social Work or related field.
• Minimum two years experience as a case manager or in a related position with children and families.
• Knowledge of and experience working with African immigrant and refugee communities.
• Strong ties to African immigrant, refugee, or diaspora communities and ability to build trust with children, families and stakeholders.
• Fluency in Swahili, French, Amharic, Somali, Arabic or other relevant languages preferred.

Responsibilities

• Identify resources within the Philadelphia community (MH/MR centers, schools, day care, health centers, housing, legal aid) and maintain up-to-date records of resources.
• Establish ties with West-African refugee groups and leaders.
• Assess service needs of refugee children and families and develop individualized plans with families.
• Link refugee children and families to needed resources and remove participation barriers (e.g., insurance).
• Maintain written/electronic records of client and resource contacts.
• Organize, run, and document Multicultural Community Events.
• Participate in staff trainings and meetings.
• Collaborate with the care team to ensure needs are met and interventions implemented.
