• Requires vocational/technical training in social work or related field.
• Bachelor’s degree in Social Work preferred.

Responsibilities

• Provide trauma-informed, harm-reduction care coordination on Flourish Mobile Health Clinic.
• Coordinate behavioral, primary, and specialty health referrals.
• Deliver crisis intervention and connect patients with community resources.
• Engage in community outreach and coordinate on-site experiences for students.
• Maintain accurate and confidential patient records.
• Respond to emergency situations including mental health.
