Job summary

Heluna Health is seeking a Care Coordinator to provide strengths-based, trauma-informed case management to individuals experiencing homelessness and behavioral health crises in Contra Costa County. The role uses harm-reduction approaches and coordinates with local partners to stabilize housing and connect clients with services.

Qualifications

• Education: Bachelor’s in Public Health, Counseling, Social Work or related field.
• Experience: 2 years direct experience with homelessness or serious mental illness.
• Substitution: AA degree with 3 years housing case management or HS diploma/GED.

Responsibilities

• Respond to dispatches across Contra Costa County.
• Conduct comprehensive assessments: medical, psychosocial, safety, substance use, housing needs.
• Develops individualized service plans with Contra Costa Continuum of Care.
• Provide crisis counseling and motivational interviewing.
• Accompany participants to appointments and services; navigate systems.
• Maintain case notes, records, and data for reporting.
