Qualifications

• Bachelor’s degree in health or human services.
• Minimum 1 year in health or human services; 2 years case management preferred.
• BLS certification required within 90 days of hire.

Responsibilities

• Collaborate with the treatment team to design and implement patient care plans.
• Assist patients in accessing services and resources to meet healthcare needs.
• Provide trauma-informed, patient-centered support to improve quality of life.
• Document care planning and progress, and communicate with providers.
