• 1+ year of human service-related experience preferred.
• Experience with substance abuse and mental health case management.
• Knowledge of patient health information and HIPAA compliance.

Responsibilities

• Provide intensive case management to incarcerated patients.
• Initiate services during incarceration and plan transition to community care.
• Coordinate care with a multidisciplinary jail-based team.
• Deliver care coordination and care transition services.
