Case Manager, Durango Integrated Healthcare, Durango, COPosition Summary: We are seeking a Case Manager who will participate as a member of a treatment team to design and implement care plans for patients with a wide range of behavioral health conditions. Assists patients in accessing healthcare services and resources to meet healthcare needs, utilizing a trauma-informed, patient centered approach focusing on patient identified goals. Assists patients in improving overall quality of life through improvement in health, relationships, meaningful activities and access to resources.Qualifications: Our ideal candidate for our Case Manager position will maintain a Bachelor’s degree in health or human service-related field;. Additionally, they will have a minimum of 1 year in health or human services setting; Preferred 2 years case management experience.
