Job summary

Parkland Health is seeking a transplant nurse coordinator to manage pre- and post-transplant patient care from referral through follow-up. You will educate patients, coordinate testing, and liaise with dialysis centers, Parkland staff, UT Southwestern, and other providers to ensure optimal outcomes.

The role requires at least 2 years of RN clinical experience (transplant preferred), a nursing degree, and a current Texas license or compact license, plus BLS certification.

Qualifications

• Must have a nursing degree from an accredited school.
• Must have 2 years RN clinical experience; transplant preferred.
• Current, valid license from the Texas Board or a Compact RN license.
• Current BLS for Healthcare Providers certification from AHA/ARC/Military Training Network.

Responsibilities

• Coordinate pre/post-transplant patient care across all phases of the transplant process.
• Educate patients and coordinate evaluation testing and record reviews.
• Communicate with dialysis centers and prepare for selection committee presentations.
• Provide post-transplant follow-up care with dialysis units and Parkland/UT Southwestern staff.
• Triages calls, schedules appointments, and educates patients on signs of rejection and hospital policies.
• Implement discharge plans, coordinate home health, and ensure complete medical records.
• Assist physicians, locate lab results, and schedule follow-ups to optimize clinic flow.
• Identify opportunities to improve transplant OPC department processes and participate in QAPI activities.
