Job summary

Barnes-Jewish Hospital in conjunction with Washington University Medical Center seeks a Transplant Coordinator to manage pre- and post-transplant care and coordinate with physicians and the care team. The role emphasizes patient education, discharge planning and follow‑up care within the transplant program.

Onsite work is required with a nursing orientation; RN licensure is required and transplant experience is a plus. A strong interdisciplinary approach is essential to success in this role.

Qualifications

• Nursing diploma or associate degree required.
• RN license (registered nurse).
• Bachelor's degree in nursing preferred with transplant experience beneficial.

Responsibilities

• Manages health care of transplant patients and donors.
• Coordinates care with physicians and health care team.
• Provides education throughout continuum of care.
• Documents assessment, plan of care, implementation and evaluation.
• Collaborates with interdisciplinary team to resolve problems in patient care.
