Qualifications

Responsibilities

• Provide direct nursing care to nephrology and post-transplant patients and families.
• Prepare patients for physician examinations and obtain accurate vital signs.
• Collect and document medical histories and other health information.
• Perform EKGs and administer medications/injections as ordered.
• Perform TB skin testing per established protocols.
• Review medications with patients and document in medical records.
• Act as patient advocate and support continuity of care with follow-up.
• Educate patients and families about care plans and treatments.
• Assist with laboratory review alongside physicians/PAs.
• Coordinate prescription refills per established protocols.
• Participate in interdisciplinary post-transplant care planning.
• Follow infection control policies and maintain clinical documentation.
• Support a collaborative clinic environment.
• Maintain exam rooms, supplies, and clinical logs.
• Assist with ordering medical supplies and medications.
• Participate in staff meetings and ongoing professional development.

We’re hiring a Registered Nurse to join a specialty clinic supporting patients receiving nephrology and post-transplant care. This is a great opportunity for an experienced RN with a strong background in post-transplant patient care who enjoys patient education, care coordination, and working closely with providers.

Responsibilities

- Provide direct nursing care to patients and families receiving nephrology and post-transplant care

- Prepare patients for physician examinations and obtain accurate vital signs

- Collect and document medical histories and other relevant health information

- Perform EKGs and administer medications/injections as ordered

- Perform TB skin testing according to established protocols

- Review medications with patients and provide accurate documentation in the medical record

- Serve as a patient advocate and support continuity of care through appropriate follow-up

- Provide patient and family education related to care plans and treatment

- Assist with laboratory review alongside physicians/PAs

- Handle patient and family phone calls, provide follow-up, and relay messages to providers as needed

- Coordinate prescription refills according to established protocols

- Participate in interdisciplinary care planning for post-transplant patients

- Follow infection control policies and maintain accurate clinical documentation

- Support a collaborative and professional clinic environment

- Maintain exam rooms, supplies, medication samples, and required clinical logs

- Assist with ordering medical supplies and medications as needed

- Participate in staff meetings and ongoing professional development

Requirements
