Job summary

Baylor St. Luke’s Medical Center in Houston, TX is seeking an RN Transplant Coordinator to guide patients from referral through recovery. You will coordinate referrals, evaluate candidates, lead inpatient care, and manage education and communications with payers and teams to ensure smooth transplant pathways.

Qualifications include a Bachelor of Nursing, TX RN license, and 3+ years of experience, with preferred acute care/ICU or transplant exposure. This is a full-time, in-hospital role.

Qualifications

• Bachelor's degree in Nursing required at hire.
• Three years of experience required at hire.
• Acute Care/ICU and/or transplant experience preferred.
• Registered Nurse licensed in TX at hire.

Responsibilities

• Initiate referrals and evaluations for transplant candidates.
• Manage in-hospital patient care and coordinate postoperative processes.
• Communicate with payers and referring physicians to avoid denials.
• Lead pre-op and post-op education and ensure language and learning needs are met.
• Provide staff and community education on transplant and organ donation.
• Collaborate on research protocols and educational materials.
