Qualifications

Responsibilities

• Coordinate education and care for transplant patients pre- and post-transplant.
• Manage a caseload of transplant patients and donors.
• Coordinate referrals and address roadblocks in transplant workup.
• Provide patient, family, and staff education.
• Ensure compliance with policies and regulatory requirements; stay in touch with governing agencies.
• May provide outreach services to regional dialysis and nephrology programs.
