Job summary

Mayo Clinic in Minnesota is seeking a Transplant Coordinator Nurse to oversee care coordination for transplant patients. You will collaborate with patients, families, and a multi-disciplinary team to plan and implement care across the continuum.

The role requires an ACEN/CCNE-accredited nursing program graduate, an RN license, and 1 year of clinical RN experience (3 years preferred). Sponsorship may be available; on-site work in Minnesota.

Qualifications

• RN license by state requirements.
• Graduate of ACEN or CCNE accredited nursing program.
• Minimum 1 year of clinical RN experience; 3 years preferred.

Responsibilities

• Coordinate all aspects of care for transplant patients.
• Assess and plan with patient, caregiver, and multidisciplinary team across the care continuum.
• Provide patient and family education on a consistent basis.
• Lead through community involvement, committees, and quality improvement and research.
• Follow ANA, UNOS, and FACT guidelines in practice.
