UNC Medical Center in Chapel Hill, NC is seeking a transplant social worker/case manager to provide comprehensive social work assessments and care coordination across pre- and post-transplant phases. You will educate patients and families, coordinate discharge planning, and participate in regulatory compliance and quality improvement activities.

The role requires an MSW, NC LCSW preferred, with 3 years of social work experience; transplant experience is a plus.

Qualifications

• Master’s degree in Social Work required.
• Licensed Clinical Social Worker (LCSW) in NC preferred; CCTSW encouraged within 3 years.
• Three years of social work experience; transplant experience if possible.

Responsabilités

• Conduct comprehensive social work assessments for transplant candidates and donors.
• Coordinate care plans across pre-, peri-, and post-transplant phases.
• Educate patients and families on transplant processes, adherence, and post-discharge needs.
• Participate in discharge planning, care transitions, and quality improvement activities.
• Communicate with payers and comply with CMS, OPTN, TJC, and NC transplant regulations.
