Job summary

Qualifications

• Master’s degree in Social Work
• Three (3) years post Master’s experience in a healthcare setting preferred
• Bilingual preferred

Responsibilities

• Completes in-depth transplant evaluations to determine candidacy for transplant.
• Provides short-term counselling/treatment to patients and families throughout the transplant process.
• Collaborates with multi-disciplinary transplant team on discharge planning for inpatients.
• Links transplant patients with Medication Assistance Programs and other community referrals.
• Performs documentation and reporting requirements.
• Educates, trains, and supervises MSW students during internship.
• Assesses living donors for donation candidacy and follows up post-surgery.
