Job summary

Hoag Memorial Hospital Presbyterian in Irvine, CA is seeking a Care Manager Transplant Social Worker (MSW) to provide psychosocial assessment, intervention, and care coordination across the transplant continuum. Responsibilities span outpatient evaluation, waitlist management, inpatient care, discharge, and post-transplant follow-up.

The role requires an MSW, CA-licensed social worker (LCSW) and transplant-related experience; certification (CCTSW) is preferred.

Qualifications

• Master's degree in Social Work (MSW) required from an accredited school of social work.
• Minimum of three (3) years of clinical social work experience in a healthcare setting.
• Experience conducting comprehensive psychosocial evaluations, including assessment of mental health, substance use, adherence, caregiver support, coping, financial barriers, and social determinants of health.
• Experience with solid organ transplantation or related complex specialty preferred.

Responsibilities

• Conducts comprehensive pre-transplant psychosocial evaluations to assess transplant readiness and identify factors affecting candidacy or post-transplant outcomes.
• Collaborates with physicians, surgeons, transplant coordinators, and other team members to develop and implement individualized plans of care.
• Provides ongoing psychosocial support and intervention throughout the transplant journey, including counseling and crisis intervention.
• Maintains timely documentation of psychosocial assessments, interventions, and follow-up plans; supports program quality improvement and CMS/OPTN compliance.
• Supports transplant program operations as a psychosocial resource and acts as a mandated reporter for suspected abuse.
