Merakey in Cambria County, PA, is seeking a Blended Case Manager - Transition Team to join our dedicated team. The role focuses on developing, coordinating and implementing services per the client service plan, serving as primary contact for up to 30 individuals and providing supportive counseling.

Qualifications include a Bachelor’s degree in social sciences, or RN, or a HS diploma with 12 credits in social sciences and two years of direct mental health experience.

Position Details

Under the direction of the Blended Case Management Supervisor, this position will be responsible for developing, coordinating and implementing services for consumers as identified in the service plan. As a member of the BCM team this position will serve as primary contact for no more than 30 individuals. Core responsibilities will include providing case-management, service planning and coordination of services with other agency providers to meet the clients' needs along with supportive counseling, and will include on-call responsibilities.

The ideal candidate will possess the following qualifications:

• Bachelor's degree with major course work in sociology, social welfare, psychology, gerontology, anthropology, other related social sciences, criminal justice, theology, nursing, counseling, or education; or,
• Registered nurse; or
• A high school diploma and 12 semester credit hours in sociology, social welfare, psychology, gerontology or other social science and two years' experience in direct contact with mental health consumers
