Job summary

Merakey in Dubois, PA is seeking a full-time Case Manager to join the Dual Diagnosis Treatment Team. The role focuses on assessing, planning, and transferring recovery skills for adults and youth with mental health diagnoses, including autism, in diverse settings.

You will work with the treatment team to implement person-centered plans, provide intensive therapeutic services, and support growth within Merakey's adult behavioral health program.

Qualifications

• Bachelor's degree in Social Work, Psychology, or another human service-related field is required.
• Will be required to complete clearances and background checks, at Merakey's expense.
• Valid PA driver's license with a clean driving record is required.
• 1 year of experience in the behavioral health field is required.
• Candidate must be open to community-based travel.

Responsibilities

Position Details

The Case Manager is responsible for assessing, planning, and skill transferring recovery treatment and support plans for adults and youth with a mental health diagnosis and/or on the autism spectrum. They provide intensive therapeutic services in all types of settings, including but not limited to community living arrangements, schools, community settings, natural support residents, etc. This candidate, in collaboration with other members of the treatment team, implements and transfers skills identified in the recovery treatment and support plan that utilizes a person-centered approach.
