Merakey is seeking a Blended Case Manager for our program in Philadelphia, PA. The role focuses on developing, coordinating, and implementing services for assigned consumers as outlined in their service plans, addressing behavioral and physical health, independent living, education, and socialization.

The BCM will work to support families and individuals, leveraging Merakey's extensive benefits package to promote well-being, financial security, and work-life balance.

Qualifications

• A bachelor's degree in psychology, social work, nursing, rehabilitation, counseling, or other related human services field.
• Experience in the human services field working with children is preferred.
• Must have a valid driver's license and own a reliable means of transportation.
• Bi-lingual a plus.

POSITION OVERVIEW

The Blended Case Manager (BCM) position is responsible for developing, coordinating, and implementing services to assigned consumers as identified in their service plan. The BCM will work and assist their assigned consumers by focusing on the following life domains: behavioral and physical healthcare needs, family functioning/independent living, educational and vocational goals, and socialization.
